The Role of the PONT

The Role of the PONT in Caring for Babies Discharged from the Neonatal Unit

The Paediatric Outreach Nursing Team (PONT) provides a 7 day nursing service to children with a wide range of nursing and medical needs. Our input to the children and their families may range from a supportive telephone call, a one day visit to complete a treatment of intravenous Antibiotics at home to months and even years of visits for children with complex and chronic conditions. These could be children with eczema and asthma and of course ex-premature babies who require additional Oxygen at home and assistance in their feeding through a Naso-gastric or Gastrostomy tube. We are all experienced nurses and have great experience in hospital and community nursing. Our role is varied and can include;  providing families with equipment, accessing other services, working with the childs school or nursery providing teaching and support. We work closely with the multi-disciplinary team and in partnership with the family. Overall we aim to provide holistic care to empower the families to lead an as normal life as possible.
